You could create a simple one using a single Miro table object. Rather than look for a “canned” template, it would be better to work with your stakeholders to understand their form & structure needs and create something which fits those.

Otherwise, you’ll end up spending as much time converting a “canned” template to fit your organization’s branding, theme, information needs…

@Michael Southcott FYI, Kiron is a purist! He always tells people to do stuff from scratch instead of using templates, haha. Anyhow, I’ve created a template and submitted it to Miroverse. While it gets reviewed, you can use it with this link (I’ll send the password in a DM):
